We are so glad you have chosen Construction Kids Preschool for your child’s early years!  Now that your child’s preschool application has been accepted, please follow the steps below before the first day.

View The Welcome Packet

The welcome packet includes information on payment, drop-off, pick-up, sick policies, health policies, and more! Read through the packet to familiarize yourself with the information.

Read The Parent Handbook

Parents of all students are required to read through our Handbook to become familiar with our policies and procedures and provide school supplies for their child. Read our Parent Handbook BEFORE beginning the required forms.

Printed copies of the handbook are available for $10. Contact us after August 1 to request the most recent version.

Setup Procare Parent Portal

Click the link below to setup your Procare Parent Portal. This portal allows you to pay for classes, manage your child's information, message the teachers, and more! Please contact the Admissions Coordinator if you are having issues accessing your account.

Complete Necessary Forms

All students are required to submit the Required Enrollment Forms below each year even if your child previously attended CKP. Only new students need to submit a copy of their birth certificate (passport can be accepted). There are additional forms that apply to certain students as well. Please continue below to submit these forms if you have received confirmation that your child has been accepted. If you have any questions, please let us know.

View Class Supply List

Supplies can be brought in during the Meet the Teacher event in August or your child's first day of school, whichever comes first. Inform your child’s teacher at the beginning of preschool if you are unable to locate certain items. We appreciate your support!

Due Dates

If you are enrolling for a new preschool year that begins in September, please wait until July 1 to begin forms to make sure you have the most up-to-date forms and policy information. As a reminder, our preschool year is similar to a traditional school year (September through May). The forms and parent handbook are updated each summer to prepare for the next preschool year. If enrolling late, please submit these forms on or before your child’s first day of preschool.

Online & Hard Copy Forms

Required forms must be completed by a parent or legal guardian of an enrolled student.

Online forms are signed electronically by entering your legal first and last name and submitting the form. Only enter initials where indicated. Incomplete forms will not be accepted.

Hard copy forms may be submitted in-person OR by mail. Contact us to schedule a time to drop off forms if you prefer to do so in-person.

Important Reminder!

These forms are due annually even if your child previously attended CKP. The one exception is the child’s birth certificate, which is only required the first year. Also, please wait for notification that your child has been accepted into our program before completing these forms.

Forms For All Students

All students, new and returning are required to submit the online form in this section. Carefully read through each section below, including CKP’s Parent Handbook, BEFORE completing the form in this section.

Important Immunization Info

Already have a copy of your child’s most recent Immunization Record? Upload a digital copy of it to the form in this section. If you only have a hard copy of the record, we suggest downloading a scanning app to your device to create a digital copy. Doctor’s offices may fax the record to us at 844-778-7072, if you prefer.

Requesting an Immunization Exemption or Alternate Immunization Schedule? Upload your letter for a Religious Exemption or Alternate Immunization Schedule to the form in this section. Make sure it follows all NC requirements or we will not be able to accept it, which could delay your child’s start date. Medical Exemptions must be signed by a physician, therefore those cannot be uploaded digitally. More details are in the form.

CKP will not accept Exemption Letters for nonreligious and non-medical reasons. If you are not vaccinating your child and have no religious or medical reason for it, your child may be withdrawn from our program.

Forms For New Students

All students, new and returning are required to submit the online form in this section. Carefully read through each section below, including CKP’s Parent Handbook, BEFORE completing the form in this section.

Submit Birth Certificate

New students must submit a copy of the child’s original birth certificate (passport is also accepted). Submit the document in-person OR by mail to the preschool office. If you need CKP to copy the form for you, contact the office to schedule a day to bring this by on or before August 15, or your chid’s first day of school, whichever comes first.

Please note that if you have not yet ordered a birth certificate it can take up to five weeks in NC for one to be delivered via mail. If your child was born in NC, same day service is available at the NC Vital Records or Register of Deeds in the child’s county of birth. Please contact us if you are still waiting on your certificate.

Forms for Certain Students

Only certain students are required to submit the forms in this section. Please carefully read through this section to see if any of these apply to your child.

Transitional Kindergarten Students

Parents of students in CKP’s Transitional Kindergarten classes must complete and submit the TK Parent Observational Checklist.

Infant Students

Parents of students in CKP’s Infant Classes must complete and submit the Infant Sleep and Feeding Policies and Plans to help your child’s teachers know how to best serve their students.  Parents are responsible to update these forms throughout the year, as needed.

We lay babies babies on their backs to sleep.  The below form must be completed by a physician ONLY IF your child is less than six months old AND you request for him/her to sleep in a different position.  This form may only be submitted in person OR by mail.

Action Plans for Asthma, Diabetes, Food Allergies, and Seizures.

Your child’s physician should complete the applicable “Action Plan” ONLY IF your child has Asthma, Diabetes, Food Allergies, or Seizures. These will help us provide better care for your child in an emergency.

Over-The-Counter & Prescription Medication Form

This form must be completed by a physician ONLY IF your child must take medication while in our care, or may require medication, such as Benadryl, in case of an emergency.

How Do I Send Medication?

Children requiring emergency medication while at preschool will typically need both the Over-the-Counter & Prescription Medication form along with the appropriate Action Plan form. Please contact us if you are unsure about which forms your child may need.

All of the medication forms must be printed. Hard copy forms in this section must be submitted with original signatures. Both a physician AND a parent or legal guardian must sign these forms. Bring the properly completed forms to our office to drop off in person OR mail them to our office.

Pay special attention to the information regarding medications. Medication must not expire during the preschool year and should be in the original container. Prescription medications must have a pharmacy label attached.

If a child’s medical needs change during the year, parents are responsible to inform the preschool and submit new forms and provide the proper medication.

Should I Send My Child's Medication?

If your child has been diagnosed with a medical condition that may require emergency medication, then you will be required to send in the medication and forms signed by a physician at the beginning of your child’s enrollment. Even if the condition is mild and/or rarely requires medication, it is still required.

It is impossible to know exactly when your child may need his/her medication, therefore, we require that it be on campus in case of an emergency. For example, if your child with asthma typically only needs his/her inhaler in the winter, CKP requires that the medication be submitted at the beginning of your child’s enrollment.
