Welcome to Our Preschool Family!
We’re so glad you’ve chosen Construction Kids Preschool for your child’s early years! Now that your child’s preschool application has been accepted, we need the following forms completed before your child can attend. Please print, properly complete and submit all necessary forms to the preschool office between July 15th and August 15th. If enrolling late, please submit these forms on or before your child’s first day of preschool.
If you are enrolling for a new preschool year that begins in September, please wait until July 1 to print and complete the below forms to make sure you have the most up-to-date forms and policy information. As a reminder, our preschool year is similar to a traditional school year (September through May). The forms and parent handbook are updated each summer to prepare for the next preschool year.
Important Note: These forms are due annually even if your child previously attended CKP. The one exception is the child’s birth certificate, which is only required the first year. Also, please wait for notification that your child has been accepted into our program before completing these forms.

All students are required to submit the items listed below. Click on the name of each document to download and print. Hard copy forms must be submitted in person or by mail, with the exception of the physician’s immunization record, which may be faxed to our office. Please read through the list to make sure you have properly completed all the necessary forms.
Child’s Birth Certificate (Photocopy) – Please do not submit a hospital’s “Mother’s Copy of Birth Certificate.”
Immunization Record – Note: You may request from your doctor a copy of your child’s immunization record. However, you must still print, sign and submit this document. Doctor’s offices may fax the form to us at 844-778-7072.
Tuition Addendum – Applies to Late Enrollees Only (enrolling after August 1). This form is only available from the Admissions Coordinator.
Additional Medical Forms
Only students with certain medical needs are required to submit the following forms, which must be signed by a physician AND parent or guardian. Please carefully read through this section to see if any of these apply to your child.
Over-the-Counter & Prescription Medication – This form must be completed by a physician ONLY IF your child must take medication while in our care, or may require medication, such as Benadryl, in case of an emergency.
Medical Action Plans – Your child’s physician should complete the applicable “Action Plan” ONLY IF your child has Diabetes, Seizures, Asthma or Allergies. These will help us provide better care for your child in an emergency.
Only students in certain age-levels are required to submit the forms in this section. Please carefully read through this section to see if any of these apply to your child.
Alternative Sleep Position Forms – We put babies down to sleep on their backs. If you desire the teachers to put your child down in a different position, please complete the appropriate form for the child’s age. The “Doctor Request” form must be completed by a physician for children under 6 months old. The “Parent Request” form only needs a parent signature and is for children 6 months old and older.
Alternative Sleep Position – Doctor Request – Only required if you want the teachers to NOT put your child down on his back to sleep AND your child is under 6 mos. old. A physician’s signature is required on this form.
Alternative Sleep Position – Parent Request – Only required if you want the teachers to NOT put your child down on his back to sleep AND your child is 6 mos. old or older. Only a parent’s signature is required on this form.
Infant Feeding Plan – Update this form throughout the year as needs change.

Printed copies will be available at Parent Orientation. Late enrollees may pick up their printed copy from the Admissions Coordinator.
